District Conference 3 ~ Online Conference Forms

This is to assist the Club Presidents in completing some of the registration forms for the conference. After completing the form, make sure to print from the "completed page" for your records.

At each District Conference, clubs are encouraged to reserve their tables early and donate a raffle basket(s). Zonta Store and Raffle Basket(s) Form

Each club must submit a Memorial Service Form even if it is fortunate enough not to have lost any members this past year. Memorial Services Form

Each club is entitled to one (1) delegate and one (1) alternate. If a club’s membership exceeds forty (40) members, the club is entitled to two (2) delegates and two (2) alternates. If a club has over eighty (80) members, it is entitled to three (3) of each. Please note this form has been designed to allow the submission of a delegate(s) and alternate(s). When completing this form you must indicate in the required field a number between zero and 3 for each category, delegates and alternates. Delegate and Alternate Form

If your club does not have a delegate attending make sure your vote counts. Contact another club and request that its delegate carry your club's proxy before completing this form. clubs electing to be represented by proxy shall be entitled to one (1) proxy regardless of the number of members. No club may carry more than two (2) proxies. No club may be represented by proxy at more than two (2) consecutive District Conferences. Proxy Form
